Medical Social Worker REPORTS TO:
Nursing Supervisor or Designee FLSA:
Exempt JOB SUMMARY Provide wide range of psychosocial services to terminally ill patients and their families, including regularly assessing the patient and family psychosocial needs, and anticipatory grief needs.
Implements the Plan of Care provisions to meet their needs, which include social service goals for alleviating problems, supportive counseling, problem solving, and community referrals for additional services, as needed.
Regularly re-evaluates the patient's and family's psychosocial needs.
Adheres to Company policies and procedures and regulatory compliance requirements and directives.
Manage related expenditures in a fiscally responsible manner in accordance with the Company's budget.
Travel as necessary.
QUALIFICATIONS 1.
Minimum age requirement of 18.
2.
Master's degree in social work.
Master's degree from school accredited by the Council on Social Work Education required in states as specified.
3.
Licensure as required by State (i.
e.
NJ requires LSW or LCSW).
4.
Minimum of two year's social service experience in a health care setting or social services agency, preferred.
5.
Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.
).
6.
Compliance with accepted professional standards and practices.
7.
Understands Hospice philosophy, principles of death/dying.
8.
Ability to work in an interdisciplinary setting.
9.
Physical exam in accordance with state law requirements.
10.
Valid driver's license and an automobile that is insured in accordance with state and/or Vitality Hospice requirements and is in good working order.
11.
Satisfactory references from employers and/or professional peers.
13.
Ability to work within an interdisciplinary setting.
14.
Demonstrate excellent observation, judgment, and communication skills.
Ability to provide good written documentation in a timely manner.
15.
Self-directed with the ability to work with little supervision.
RESPONSIBILITIES 1.
Work within the scope of the position, in coordination with management, to meet the needs of our patients, families and professional colleagues.
2.
Conduct initial psychosocial assessment of patient/family/caregiver in a thorough, accurate, compassionate and timely fashion.
3.
Function as a support person for the patient/family/caregiver on an ongoing basis.
4.
Function as expert and facilitator of obtaining community resources to meet the needs of patients and families, including financial assistance.
Assist families with referrals to community resources when indicated.
5.
Assist family with Advanced Directives.
6.
Assist family/caregiver in placement of patient in a facility for basic care or respite, when appropriate.
7.
Complete and maintain all patient records at patient's residence and submit accurate and relevant notes regarding patient's condition and care given.
8.
Provide psychosocial assessment and counseling to patients and family/caregiver.
9.
Attend and actively participate in team meetings, staff meetings and in-services, and contribute positively to patient care planning.
10.
Conduct pre-bereavement and bereavement visits or contacts and/or support groups as appropriate.
11.
Work in concert with other Vitality Hospice interdisciplinary team members.
12.
Provide and maintain a safe environment for the patient.
13.
Assist patient/family/caregiver and other team members in providing continuity of care.
14.
Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
15.
Maintain clinical records, statistics, reports and records for purposes of evaluation and reporting of agency activities as prescribed and in compliance with local, state and federal laws.
16.
Maintain a caseload and deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
17.
Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
18.
Maintain all appropriate state licensing requirements.
19.
Actively participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.
20.
Use supplies and equipment effectively and efficiently.
21.
Regular attendance is to be maintained.
22.
Adherence to a code of conduct conducive with Vitality Hospice HealthCare policy is expected.
23.
Meet or exceed delivery of Company Service Standards in a consistent fashion.
24.
Interact with all staff in a positive and motivational fashion supporting the Company's mission.
25.
Conduct all business activities in a professional and ethical manner.

WORKING CONDITIONS General office, extended care facility, hospice inpatient unit, hospitals, group homes, residential care facilities, patient home setting, exposure to infectious diseases, travel by automobile and occasionally by airplane.
PHYSICAL DEMANDS While performing the duties of this job, the employee is regularly required to stand, sit, walk, reach with hands and arms, twist, stoop, kneel, crouch or crawl.
Regularly required to use hands, talk or hear.
Must be able to travel to businesses, offices, or patient homes or where conditions may be adverse due to weather, environment, or patients' lifestyles.
